Copy it exactly from the passport.
Given Name
Enter every given name in passport order.

Enter it without extra symbols or spaces.

Enter the issued visa or the applicable visa-exempt basis.
Port of Entry
Select the airport, seaport or land checkpoint actually used.
Flight / Vehicle Number
Use the final service that brings you into Cambodia.
Purpose of Visit
Select Tourism or Holiday for a leisure trip.
Address in Cambodia
Enter the first hotel or accommodation.
Health Declaration
Answer truthfully about symptoms and travel history.
Customs Declaration
Declare relevant cash, goods and baggage accurately.
After submission
Save the QR code offline and present it to immigration and customs. The declaration does not replace a visa or entry permission.
