First / Middle / Last Name / Suffix
Copy the passport and do not invent absent name parts.
Passport Number
Enter without extra symbols or spaces.
Sex / Birth Date / Civil Status
Enter accurate details.
Nationality / Country of Residence
These are separate fields; complete each accurately.
Date of Arrival / Port
Enter the actual airport or seaport.
Airline / Vessel / Flight Number
Enter the final segment into the Philippines.
Traveller Type / Purpose
A foreign tourist selects the applicable foreign-national and holiday or pleasure options.
Destination Address
Enter the first hotel or host address in the Philippines.
Health / Customs Declaration
Answer the current system questions accurately.
After submission
Save the QR code and reference number. Before border processing, some details can be changed through Edit Registration using the reference number and birth date.
